FDA Grants Breakthrough Status to Blood Test That Distinguishes Bipolar From Schizophrenia

A new mRNA biomarker test from Laguna Diagnostics achieved 98.3 percent accuracy in telling the two conditions apart, using a simple blood draw.

The U.S. Food and Drug Administration has granted Breakthrough Device Designation to a blood test that can help distinguish between schizophrenia and bipolar I disorder. The announcement, made on April 28, 2026, by Laguna Diagnostics, marks a significant step toward bringing objective diagnostic tools into psychiatry, a field that has relied almost entirely on subjective clinical assessment.

The test works by analyzing mRNA biomarker signatures from a simple venous blood draw. It generates an objective probability score designed to help clinicians differentiate between the two conditions. In clinical testing, the results were striking: 96.7 percent sensitivity for schizophrenia, 100 percent specificity for bipolar I disorder, and 98.3 percent overall accuracy.

The test is not intended as a standalone diagnostic. It is designed for use alongside clinical assessment and other patient information, adding an objective data point to what has traditionally been a subjective process.

Why This Matters

Schizophrenia and bipolar disorder can look remarkably similar in their early stages, particularly when psychotic symptoms are present. Research suggests that misdiagnosis rates for these conditions may exceed 50 percent, and current methods often require one to three or more years of observation before clinicians can reach a confident diagnosis.

That delay has real consequences. Treatments for the two conditions differ significantly. Patients who receive the wrong diagnosis may spend years on medications that do not address their actual condition, leading to preventable hospitalizations, worsening symptoms, and damaged relationships.

For people with bipolar disorder specifically, the path to an accurate diagnosis already takes an average of nearly a decade. A tool that can provide objective differentiation from schizophrenia early in the process could shorten that timeline significantly.

What Breakthrough Device Designation Means

The FDA’s Breakthrough Device Designation is reserved for technologies that may provide more effective diagnosis or treatment for life-threatening or irreversibly debilitating conditions. The designation provides Laguna Diagnostics with priority review and closer collaboration with the FDA as the test moves toward potential approval, but it does not constitute FDA approval or clearance of the device.

The test is part of a broader trend in psychiatry toward biological markers that can supplement clinical judgment. While psychiatric diagnosis has historically depended on symptom observation and patient reporting, advances in genomics, brain imaging, and now mRNA analysis are beginning to offer clinicians objective tools that could reduce diagnostic uncertainty.
