A new study found that people experiencing their first manic episode had significantly lower cholesterol and triglycerides than healthy individuals, and lower levels correlated with more impulsive behavior.
A study published in Cureus has found a significant link between low cholesterol levels and impulsive behavior during first-episode mania. The research, conducted at SCB Medical College in Cuttack, India, compared lipid profiles and impulsivity scores between 60 patients experiencing their first manic episode and 60 age-matched healthy controls.
The results showed a clear pattern. Patients in the grip of their first manic episode had significantly lower total cholesterol (156.58 mg/dL vs. 175.93 mg/dL), lower LDL cholesterol (75.00 mg/dL vs. 83.58 mg/dL), and lower triglycerides (74.03 mg/dL vs. 96.43 mg/dL) compared to the healthy group. All three differences were statistically significant.
Impulsivity was measured using the Barratt Impulsiveness Scale (BIS-11), a widely used clinical tool. Higher scores on the scale correlated with lower cholesterol levels, particularly total cholesterol and LDL. In other words, the lower a patient’s cholesterol during mania, the more impulsive their behavior tended to be.
What Could Explain the Connection?
Cholesterol plays a critical role in the brain. It is a key component of cell membranes in neurons and is involved in the production and regulation of serotonin, a neurotransmitter that helps control impulses, mood, and aggression. Lower cholesterol levels have been associated with reduced serotonin activity, which researchers believe may contribute to the disinhibited, impulsive behavior that characterizes manic episodes.
This is not the first study to observe this connection. Earlier research has also found lower cholesterol in people experiencing mania compared to those in depressive or stable phases of bipolar disorder. What makes this study notable is its focus on first-episode patients, who have not yet been exposed to long-term psychiatric medications that can themselves alter lipid levels.
Clinical Implications
The authors suggest that lipid monitoring could be a useful tool in clinical settings for people at risk of bipolar disorder. While low cholesterol alone would not diagnose mania, it could serve as one additional marker alongside clinical observation, particularly in patients presenting with their first mood episode.
The study was small, with 60 patients and 60 controls, and was conducted at a single center in India. Larger, multi-site studies would be needed to confirm the findings across different populations. But the results add to a growing body of evidence that the biological mechanisms of mania extend well beyond the brain’s neurotransmitter systems and into broader metabolic processes.
